In general these are a sets of documents that allow people to make decisions for you when you’re unable to make those decisions for yourself. There’s a healthcare surrogate, which allows someone to make healthcare decision. There’s a living will for end of life decisions. There is a designation of preneed guardian which lets the court know your preference for legal guardianship. There is also a HIPAA release for the release of private medical information to certain people who are within your inner circle. In addition to those healthcare documents you should also have a durable power of attorney to allow somebody to make business and financial transactions for you when you’re unable to do that for yourself.
