As part of National Healthcare Decisions Day 2017, and in collaboration with Haven Hospice, we are offering three Advance Directives and Life Care Planning Talks on April 19, 20, and 21 in Orange Park, St. Augustine, and Jacksonville.

On April 19, from 1 – 3 p.m., Alison E. Hickman, J.D. presents at Haven Hospice in Orange Park at 741 Blanding Boulevard.

On April 20, from 10 a.m. – 12 p.m., Grady H. Williams, Jr., LL.M. presents at Haven Hospice in St. Augustine at 2497 U.S. Highway 1 South.

On April 21, from 10 a.m. – 12 p.m., Grady H. Williams, Jr., LL.M. presents at the Offices of Grady H. Williams, Jr., LL.M. Attorneys at Law, P.A. in Jacksonville at 7807 Baymeadows Road East, Suite 402-B.

Advance Directives and Life Care Planning can be difficult and confusing. Please let us help you start the conversation, understand, and complete the paperwork for these important healthcare decisions.

We will be assisting attendees with the completion of Florida statutorily recommended Living Wills, Health Care Surrogates, and HIPPA Authorization forms. The talks will also include information and instructions about registering to be an Organ Donor in the State of Florida.
